What to try before you choose joint care

Is it time to have my joint checked?

Book an exam once soreness keeps limiting sleep, walking or your usual work. You'll need prompt care after a hard fall or sudden loss of motion. Mild soreness that's easing can often wait. I'd watch whether the ache improves each day.

The first visit doesn't commit you to treatment. You'll get an exam and hear what may be causing the soreness. Ask what may help now, how long it takes and what can wait.

What to try at home first

Ease off the task that started the soreness, but keep gentle motion. Try several brief walks instead of one long outing. Build back slowly when the joint is calmer. If it hurts more each day, stop testing it.

Strength work can support a worn knee or hip. A physical therapist can guide your first safe exercises. Don't force a sharp or catching pain. Your doctor can adjust the work if soreness lingers.

When to get help quickly

Seek urgent care for a fever along with a hot, swollen joint. Get help after a fall if it looks bent or won't bear weight. A locked joint needs a timely exam. New weakness or numbness can't wait.

Call sooner when soreness rises fast or wakes you night after night. Tell the doctor about weight loss you can't explain. These signs don't prove one cause. They may call for more than a routine office visit.

How to prepare for the appointment

Gather your medicines, old scan reports and notes from earlier visits. Write down when soreness began and which actions worsen or ease it. Include any fall, twist or recent joint procedure. You'll give a clearer account when it's on paper.

Name the activity you miss because of soreness. Ask which part of the exam supports the likely cause. Then ask what the first treatment costs and how long to try it. Deciding can wait until after the visit.

What to ask if the first care does not help

Ask whether the likely cause still fits your soreness. A later exam may differ after swelling falls or weakness becomes clearer. A second opinion gives you another exam without starting over. Bring earlier records so the visit stays focused.

For a new treatment, ask what it uses and whether it happens in a clinic or operating room. Find out who provides it and what recovery takes. You'll want the full cost before agreeing. If the answers aren't clear, take more time.

Notably it does NOT strongly recommend any biologic injectable.

The Cochrane review of exercise for knee osteoarthritis found high-quality evidence that land-based therapeutic exercise provides short-term benefit in pain and physical function, sustained for at least 2-6 months after the programme ends, with mild transient soreness the only reported adverse effect across 45 trials. It is the best-evidenced treatment for this condition and it costs nothing per injection.

It is not approved for osteoarthritis. The existence of one licensed cartilage cell therapy for focal defects is the sharpest available way to show what a licensed 'regeneration' product actually looks like, and how far it is from an injection for a worn joint.

What to ask if it does not settle
